After Hours
If you need to speak to a physician, call the office number (407-628-0448) and you will be given instructions for reaching the doctor on call. Dr. Scoma is not always the physician on call so please do not call him on his pager .

Updating Medical Information
Each time you have an appointment, there is a form that Dr. Scoma asks you to fill out regarding any symptoms that you may be having. He appreciates the effort you take in completing the form as it assists him in not missing anything that may be going on with your health care. Thus you can understand that the completing of this form is not meant to irritate you but to assist Dr. Scoma in your medical treatment. Your cooperation is appreciated.

Late arrivals and No Shows
We strive to stay on time with scheduled appointments. IF YOU ARRIVE LATE, YOU WILL BE ASKED TO
RESCHEDULE SO THAT WE MAY ACCOMMODATE THOSE WHO ARRIVE ON TIME.
IF YOU FAIL TO KEEP YOUR APPOINTMENT.
THERE WILL BE A $25.00 CHARGE IF YOU DO NOT CALL 24 HOURS BEFORE YOUR RESERVED TIME.

Insurance Authorizations
If your insurance company requires an authorization number for your visit, it is YOUR responsibility to request one from your primary physician. Please call us before coming in for your appointment to make sure that we have received the authorization. Without authorization, you cannot be seen by Dr. Scoma and you will be asked to reschedule.

Referrals
If you need us to give a referral to another facility, we will need at least 24 hours to respond.

Copying Records
There is a charge to copy your records. The charge is $1.00 a page for the first 25 pages and then $0.25 per page thereafter. We recommend that any time you need a copy of your records, you make a set for yourself before giving the copy away. This will save you money in case you ever need another copy.

Lab Requisitions
Please do not lose your lab requisitions. Sometimes the lab work needs to be done immediately. Other times we put a date on it that is prior to your next appointment. Please get the blood drawn at least a week before the scheduled appointment so we can get the results before you arrive to see the doctor. If you have your blood drawn at another doctor's office instead of at the lab, it is your responsibility to get the lab results and bring them to our office. Thank you.
